Online Application Forms


ONLINE Application for Admission Submit completed online application and registration fee of $250 and a $35 non-refundable processing fee.  This will reserve a space for the applicant.  Unsuccessful applicants will have registration fee refunded.

Application Checklist – A convenient check-off list of all the major documentation needed for the application process.  Application packets do not need to be complete at the time of application.  However, we must receive a completed Application for Admission along with a $250.00 application fee, transcripts and ISEE scores to begin the admissions process.

Recommendation Letter Form – Letters must be sent directly to the school from at least one teacher.  The second letter might come from another teacher, coach, music instructor, pastor, or other adult not related to the applicant.  This form provides a convenient place to make a recommendation, but it is not the only acceptable format.  (The person making the recommendation needs to send the letter directly to the school.  It may not be sent via anyone other than the writer.)

Immunization Record – Must be on file at your child’s school and doctor’s office. This form enables parents to easily submit the information, but other forms are acceptable.

Request for Transcript – To be signed and returned to the office at the applicant’s current school, so that transcript information (including for 7th and 8th grade) can be forwarded to St. Thomas More Academy.  For current high school transfer students, a full official high school transcript will be necessary.  Students coming from home school should also provide the information on an official transcript.
